To: Nancy W. Rach, whose last known place of residence is 6626 Bedell Road, Berlin Center, Ohio 44401 and 1053 S Park Drive, Brookfield, Ohio 44403 you will take notice that on the 19th day of March, 2013, Plaintiff, JPMorgan Chase Bank NA Successor by Merger to Bank One NA, filed a Complaint for Foreclosure in the Mahoning County Court of Common Pleas, being Case No 13 CV 749, alleging that there is due to the Plaintiff the sum of $136,331.51 plus interest at the rate of 6.60% per annum from January 8, 2010, plus late charges and attorney fees applicable to the terms of the Promissory Note secured by a Mortgage on the real property, which has a street address of 6626 Bedell Road, Berlin Center, Ohio 44401, being permanent parcel number PPN: 22-023-0-003.000.

Plaintiff further alleges that by reason of the default in payment of said Promissory Note, the conditions of said Mortgage have been broken and the same has become absolute.

Plaintiff prays that the Defendant named above be required to answer and assert any interest in said real property or be forever barred from asserting any interest therein, for foreclosure of said mortgage, marshalling of liens, and the sale of said real property, and that the proceeds of said sale be applied according to law.

Said Defendant is required to file an answer on or before the 11th day of September, 2013.
