To: Dolores Chichak: whose last known place of residence is 209 Westminster Ave., Youngstown, OH and The Unknown Heirs, their Spouses and Creditors, Legatees, and the Fiduciary of the Estate, and Spouse and Creditors, of Dolores Chichak, whose last known place of residence is: unknown, each of you will take notice that on the 25th day of February, 2013, the Plaintiff, JPMorgan Chase Bank, N.A., filed a Complaint for Foreclosure in the Mahoning County Court of Common Pleas, being Case No. 13CV 491, alleging that there is due to the Plaintiff the sum of $26,353.29 plus interest at 4.51% (variable) per annum from April 25, 2009, plus late charges and attorney fees applicable to the terms of the Line of Credit Agreement secured by a Mortgage on the real property, which has a street address of 209 Westminster Ave., Youngstown, OH 44515, being permanent parcel number Parcel ID: 48-024-0-363.000

Plaintiff further alleges that by reason of a default in payment of said Line of Credit Agreement, the conditions of said Mortgage have been broken and the same has become absolute.

Plaintiff prays that the Defendants named above be required to answer and assert any interest therein, for foreclosure of said mortgage, marshalling of liens, and the sale of said real property, and that the proceeds of said sale be applied according to law.

Said Defendants are required to file an Answer on or before the 23rd day of April, 2013.
