Swain, whose last known place of residence is 11050 Beloit Snodes Road, Beloit, OH 44609 you will take notice that on the 19th day of April, 2012, Plaintiff, JPMorgan Chase Bank, N.A., successor by merger to Bank One, N.A., filed a Complaint for Foreclosure in the Mahoning County Court of Common Pleas, being Case No. 12CV 1160, alleging that there is due to the Plaintiff the sum of $62,996.37 plus interest at 7.25% per annum from September 28, 2011, plus late charges and attorney fees applicable to the terms of the Promissory Note secured by a Mortgage on the real property, which has a street address of 11050 Beloit Snodes Road, Beloit, OH 44609, being permanent parcel number PPN: 16-145-0-009.000

Plaintiff further alleges that by reason of a default in payment of said Promissory Note, the conditions of said Mortgage have been broken and the same has become absolute.

Plaintiff prays that the Defendant named above be required to answer and assert any interest in said real property or be forever barred from asserting any interest therein, for foreclosure of said mortgage, marshalling of liens, and the sale of said real property, and that the proceeds of said sale be applied according to law.

Said Defendant is required to file an Answer on or before the 6th day of September, 2012.
