To: Nancy W. Rach, whose last known place of residence is unknown, you will take notice that on the 19th day of September, 2011, the Plaintiff, JP Morgan Chase Bank, successor by merger to Bank One, N.A., filed a Complaint for Foreclosure in the Mahoning County Court of Common Pleas, being Case No. 11CV 3113, alleging that there is due to the Plaintiff the sum of $136,331.51 plus interest at 6.60% per annum from January 8, 2010, plus late charges and attorney fees applicable to the terms of the Promissory Note secured by a Mortgage on the real property to wit:
Known for street numbering purposes as 6626 Bedell Road, Berlin Center, OH 44401
PERMANENT PARCEL NO. 22-023-0-003.000
Plaintiff further alleges that by reason of a default in payment of said Promissory Note, the conditions of said Mortgage have been broken and the same has become absolute.
Plaintiff prays that the Defendant named above be required to answer and assert any interest in said real property or be forever barred from asserting any interest therein, for foreclosure of said mortgage, marshalling of liens, and the sale of said real property, and that the proceeds of said sale be applied according to law.
Said Defendant is required to Answer on or before the 7th day of December, 2011.
