Christopher Copeland, whose last known place of residence was 150 Benita Avenue, Youngstown, Ohio 44505, otherwise whose place of residence is unknown, will hereby take notice that on the 13th day of May, 2009, Ohio Presbyterian Retirement Services, filed a Complaint against you in the Youngstown Municipal Court of Mahoning County, Ohio, being Case Number 09CV F 1524Y on the docket of said Court, alleging that you owe the Plaintiff the sum of $5,348.88, together with interest and court costs, in connection with nursing home care provided by Plaintiff to Lisha Copeland.

You are required to answer the Complaint within Twenty-eight (28) days from the last date of publication of this notice, which will be published for six (6) successive weeks, said answer day being the 24th day of November, 2009.

In case of your failure to answer or otherwise respond as required by the Ohio Rules of Civil Procedure, judgment by default will be rendered against you for the relief demanded in the Complaint.
