TO THE DEFENDANTS, Keith D. Elliott and the unknown spouse of Keith D. Elliott, whose last known place of residence and Post Office address was 639 South 14th Street, Sebring, Ohio 44672 but whose present residence address is unknown and cannot with reasonable diligence be ascertained: Plaintiff has brought this action naming you as Defendants in the above named Court by filing its Complaint on the 19th day of November, 2008.
Situated in the Village of Sebring, County of Mahoning and State of Ohio: And known as being Lot Number Sixteen Hundred Eighty-nine (1689) in said Village of Sebring, as the same is marked, numbered and distinguished on the recorded plat of said Village in the Recorder's Office at Youngstown, Ohio. Be the same more or less, but subject to all legal highways.

And for judgment upon a Promissory Note executed to Plaintiff, the Huntington National Bank, successor by merger to Unizan Bank, National Association, by Defendant, Keith D. Elliott.
The relief demanded is a judgment against the Defendant, Keith D. Elliott, in the amount of $116,027.39, along with interest at the rate of 5.375% per annum from the 1st day of June, 2008 until paid, and for court costs; that said mortgage may be foreclosed and the said real estate described herein be sold at Sheriff's Sale and the proceeds of said sale applied for payment of Plaintiff's claim; further that Defendants, Keith D. Elliott and the unknown spouse of Keith D. Elliott, be required to answer setting forth their claim or interest in the subject real estate, if any, or forever be barred from claiming or asserting same; and for such other and further relief to which Plaintiff may be entitled.
You are required to answer the Complaint within Twenty-eight (28) days after the last publication of this notice, which will be published for three (3) consecutive weeks, with the last publication to be made on the 19th day of February, 2008.
In Case of your failure to answer or otherwise respond as permitted by the Ohio Revised Code and the Ohio Rules of Civil Procedure within the time stated, judgment by default will be rendered against you for the relief demanded in the Complaint. Answer date is March 19, 2009.
