ANTONIO L. CROCKETT, Defendant, whose last known place of residence was 627 Idora Road, Youngstown, Ohio 44511, otherwise whose place of residence is unknown, will hereby take notice that on the 25th day of August, 2004, Andrew C. Cooks, filed a Complaint against you in the Youngstown Municipal Court of Mahoning County, Ohio, being Case Number 04 CVE 3104Y on the docket of said Court, alleging that

Plaintiff, Andrew C. Cooks, demands judgment against Defendant, Antonio L. Crockett, in an amount in excess of Six Thousand Fifty Dollars and 00/100 ($6,050.00) that will fully compensate him for his injuries, losses and damages, plus interest, punitive damages, costs of this action, and any other relief this honorable finds appropriate.

You are required to answer the Complaint within Twenty-eight (28) days from the last date of publication of this notice, which will be published for six (6) successive weeks, said answer day being the 21st day of June, 2005.

In case of your failure to answer or otherwise respond as required by the Ohio Rules of Civil Procedure, judgment by default will be rendered against you for the relief demanded in the Complaint.
