Kenneth G. Calvin, whose last known place of residence is 16 Marie (Merri) Drive, North Jackson, Ohio 44451, otherwise whose place of residence is unknown, will take notice that on the 21st day of May, 2004, Joseph F. Szymanski and Carol Szymanski, filed a Complaint against you in the Court of Common Pleas of Mahoning County, Ohio, being Case Number 04CV 1750 on the docket of said Court, alleging that

On or about June 6, 2002, Plaintiff Joseph F. Szymanski was lawfully and properly operating a vehicle northbound on Orlando Drive in Canfield Township, Mahoning County, Ohio when Defendant Kenneth G. Calvin negligently operated a motor vehicle (garbage truck) in his capacity as agent, servant, and/or employee of Allison Brothers, Inc. within the course and scope of his employment; thereby causing a motor vehicle collision that resulted in bodily injuries, property damage, wage loss, pain and suffering, loss of enjoyment of life, loss of consortium and other damages to Plaintiff Joseph Szymanski and to Plaintiff Carol Szymanski. Plaintiffs Joseph Szymanski and Carol Szymanski demand judgment against Defendant Kenneth G. Calvin in excess of Twenty-five Thousand Dollars ($25,000.00) for money damages so as to fully compensate them for the aforementioned damages to person and property. Plaintiffs further seek interest, costs of this action, and all other appropriate relief.

You are required to answer the Complaint within Twenty-eight (28) days after the last date of publication of this notice, which will be published for six (6) successive weeks, said answer day being the 8th day of June, 2005.

In case of your failure to answer or otherwise respond as required by the Ohio Rules of Civil Procedure, judgment by default will be rendered against you for the relief demanded in the Complaint.
